Participation Information This web page will help you easily access important pre-participation information and forms required for Mills College intercollegiate athletics. Please take a minute to read through these instructions carefully. Forms on this site are Adobe Acrobat files. If you do not already have Acrobat Reader installed, you will need to do so to access these files. For a free download of Adobe Acrobat, visit their website. All forms are explained in the paperwork checklist. All forms and physicals must be completed and on file prior to your participation in any practice or competition. In addition to paperwork, all student-athletes must attend one physical session and one pre-participation eligibility meeting and be certified eligible by the eligibility and compliance coordinator prior to any competition. All Student-Athletes: See the Mills College Student-Athlete Handbook for all policies and important information regarding your participation in athletics at Mills. Returning Student Athletes You must have a physical exam on file, as performed by our team physician or other qualified M.D. or D.O. (physicals from chiropractors will not be accepted). Once a physical exam is on file you are required to have an annual "sports update" exam performed by the Mills College athletic trainer. An opportunity to complete this requirement on campus will be available at your team's scheduled times (see Meeting Schedules). You are responsible for completing this requirement on your own, at your own expense, if you miss the physicals on campus. New Athletes (First-year and Transfer Students) Please note that new athletes must meet both with the head athletic trainer and the team physician prior to their participation in practice or competition. New athletes are required to submit results of a physical examination from the Mills College team physician prior to your participation in Mills intercollegiate athletics. An opportunity to complete this requirement on campus will be available at your team's scheduled times (see Meeting Schedules). You are responsible for completing this requirement on your own, at your own expense, if you miss the physicals on campus. Eligibility Meetings and Physicals Attendance at one eligibility meeting is mandatory for each student-athlete, whether or not you have participated on a team previously. Attendance at the appropriate physical is also required. Monday, August 27: 10:00–11:30 am, Physicals—Cross country and tennis returning athletes 2:00–4:00 pm, Physicals—Soccer and volleyball returning athletes 5:30 pm, Physicals—New Athletes Only, Haas Pavilion, Room 114 (lobby outside the fitness center). Another session of physicals for swimming and rowing will be scheduled for early September. Tuesday, August 28: 8:00–10:00 am, Eligibility Meeting, Haas Pavilion, Room 116 Required for all new and returning athletes
Warning: Fall sport athletes who miss either their athletic physicals or the eligibility meeting will not be able to participate in practice or competition until after they complete both of these requirements. No individual meeting make-up will be scheduled outside of these times. *Note: If there are returning athletes that have existing injuries, they must be prepared to come back that evening to see the physician if they are instructed by Natalie to do so. Sign up for physicals operate on a first come, first serve basis and the athletes will be signing in upon arrival, so the later they show up, the later they will be seen. Also, the athletes may not be seen at the time they show up Coaches and Captains Please remind all student-athletes on your teams to make use of this site and the forms and instructions it contains. Changes have been made since last year, and student-athletes are responsible for compliance with the new procedures. Questions? Whew!
