Returning Athletes 

This web page will help you easily access important pre-participation information and forms required for Mills College Intercollegiate Athletics. Please take a minute to read through these instructions carefully.

Forms on this site are Adobe Acrobat files. If you do not already have Acrobat Reader installed, you will need to do so to access these files. For a free download of Adobe Acrobat, visit their website.

All forms are explained in the paperwork checklist. All forms and physicals must be completed and on file PRIOR to your participation in any practice or competition. In addition to paperwork, all student-athletes must attend one physical session and one pre-participation eligibility meeting and be certified eligible by the Eligbility and Compliance Coordinator PRIOR to any competition.

All Student-Athletes:

1. Download and print the five pre-participation documents:

2. Follow all instructions as stated on the checklist.

3. Complete and file all mandatory paperwork with the Head Athletic Trainer and Eligibility Coordinator prior to any participation (practice or competition). Paperwork to be handed in at the appropriate meeting. See Meeting Schedule for details.

4. Provide proof of current medical insurance coverage. A valid copy of your insurance card(s) must be turned in the Head Athletic Trainer prior to any participation in practice or competition. You are responsible for obtaining and submitting copies of the cards to the ATC.

5. Attend one of the two preparticipation Eligibility Meetings, as well as the appropriate Physicals for your team. See Meeting Schedule.

Returning Student Athletes

You must have a physical exam on file, as performed by our Team Physician or other qualified M.D. or D.O. (Physicals from Chiropractors will not be accepted). Once a physical exam is on file you are required to have an annual "sports update" exam performed by the Mills College Athletic Trainer. An opportunity to complete this requirement on campus will be available at your team's scheduled times (see Meeting Schedules). You are responsible for completing this requirement on your own, at your own expense, if you miss the physicals on campus.

New Athletes (Freshwomen and Transfers)

Please note that new athletes must meet both with the Head Athletic Trainer and the Team Physician prior to their participation in practice or competition. New athletes are required to submit results of a physical examination from the Mills College Team Physician prior to your participation in Mills Intercollegiate Athletics. An opportunity to complete this requirement on campus will be available at your team's scheduled times (see Meeting Schedules). You are responsible for completing this requirement on your own, at your own expense, if you miss the physicals on campus.

Eligibility Meetings and Physicals

Attendance at one eligibility meeting is mandatory for each student-athlete, whether or not you have participated on a team previously. Attendance at the appropriate physical is also required.

Schedule for (late arrival) Fall 2007 and Spring 2008 Athletes:

Tuesday, August 27: 

10:00 a.m.-noon Eligibility Meeting & intro to Athletic Training Services (Required of all athletes)

Thursday, August 29:

New Athlete Physicals:
All new athletes must have physicals with Dr. Ross

5:30 p.m. New athlete physicals for tennis, swimming, and remaining fall sport athletes

7:00 p.m. New athlete physicals for rowing

Returning Athlete Physicals:
Returning athletes will have physicals with Natalie Spangler during the following time slots:

Monday Aug. 27th 10am-12pm and 1-2pm
Tuesday Aug. 28th 9-10am and 1-2pm
Weds. Aug. 29th 10-12 and 1-2pm
Thursday Aug. 30 10-12 and 1-4pm

If you are unable to make these times, you can come between 4-6pm on any day, but you will have to wait until Natalie is available to see you.  If you have further conflicts, please call Natale Spangler at (510)430-3323.  

ALL athletes (including returners) must have their athletic training packet and physical paperwork completed, as well as a copy of their insurance card prior to showing up for physicals.

Warning: Fall sport athletes who miss either their athletic physicals or the eligibility meeting will not be able to participate in practice or competition until after they complete both of these requirements.  No individual meeting make-up will be scheduled outside of these times.

*Note: If there are returning athletes that have existing injuries, they must be prepared to come back that evening to see Dr. Ross if they are instructed by Natalie to do so.

Sign up for physicals operate on a first come, first serve basis and the athletes will be signing in upon arrival, so the later they show up, the later they will be seen. Also, the athletes may not be seen at the time they show up
and should be prepared to wait. If they do not have all of their paperwork filled out prior to arrival, they may have to wait even longer, so it is very important they have all paperwork completed.

Coaches and Captains

Please remind all student-athletes on your teams to make use of this site and the forms and instructions it contains. Changes have been made since last year, and student-athletes are responsible for compliance with the new procedures.
