There are two forms that must be completed to receive academic credit. Please note that all forms are available online in a PDF format.  They must be filled out completely, signed by all appropriate faculty, staff, advisers and internship supervisors before they are submitted to the M Center.

The forms are:

1. Petition for Academic Credit 

2. Internship Learning Contract

Please follow the instructions on the forms carefully.

Submit completed and signed Petition for Academic Credit form to the M Center by the deadline. 

Deadline: Spring, 2011 is Wednesday February 2nd at noon

Fall and Spring
File at the M Center no later than noon on Wednesday of the first full week of class in the same semester for which you are requesting credit. 

Summer 
File at the M Center no later than the official end of spring semester. 

Reminder: Most Internship Employers will require a professional style resume for application to their organization/program. Meet with a Career Services Office staff member to create the best resume.
