2013-14 Cal Grant GPA Verification Form      

       For Undergraduate California Residents Only

2013-14 California Dream Act Application

       The paper 2013-14 California Dream Act Application
       will be available on January 1, 2013.   The paperless
       (electronic version) will be available on January 14.
       Do NOT file the 2012-13 application. 

       California residents who meet the requirements of
       AB 540 and who are ineligible for 2013-14 federal
       financial aid should file the 2013-14 California Dream
       Act Application for AB 540 Eligible Students and the
       2013-14 NON-SSN Cal Grant GPA Verification Form to
       be considered for a Cal Grant and institutional (Mills)
       need-based scholarships. 

2013-14 NON-SSN Cal Grant GPA Verification Form

       California residents who meet the requirements of
       AB 540 and who are ineligible for 2013-14 federal
       financial aid should file the 2013-14 California Dream
       Act Application for AB 540 Eligible Students and the
       2013-14 NON-SSN Cal Grant GPA Verification Form to
       be considered for a Cal Grant and institutional (Mills)
       need-based scholarships. 

 2013-14 Dependent Verification Worksheet

       If you are a dependent student, and your federal
       Student Aid Report (SAR) indicates that you have    
       been selected for federal verification, please     
       complete this form and submit it to the M Center at
       Mills by May 15, 2013. 

 2013-14 Independent Verification Worksheet

       If you are an independent student, and your
       federal Student Aid Report (SAR) indicates that you  
       have been selected for federal verification, please
       complete this form and submit it to the M Center at
       Mills by May 15, 2013.

 IRS Tax Return Transcript(s)

       For new and continuing students selected for federal
       or institutional verification after filing the FAFSA.

       Students selected for verification after filing the   
       FAFSA must submit to the M Center by May 15, 2013
       Federal Tax Return Transcripts if they choose not to
       use the IRS Data Retrieval Tool (DRT) or do not
       qualify to use the IRS Data Retrieval Tool (DRT) to 
       update their FAFSA with actual 2012 tax information
       after federal tax returns have been filed.  Copies 
       of federal tax returns do not meet this federal 
       government documentation requirement.

       Students selected for federal verification are notified
       on their Student Aid Report (SAR).  Students 
       selected for institutional verification will be notified   
       by the M Center.

       Dependent students selected for verification must
       submit IRS Tax Return Transcripts of 2012 tax year  
       information for the student and parent(s).

       Independent students selected for verification must
       submit IRS Tax Return Transcripts of 2012 tax year 
       information for the student and spouse, if
       applicable. 

       Note: Students who are not selected for federal or
       institutional verification are not required to submit
       copies of federal tax returns or IRS Tax Return
       Transcripts. 

 2013-14 Federal Direct PLUS Loan Request Statement

      To be filed by parents applying for a Federal Direct
      PLUS Loan. This form should be submitted to the
      M Center.           

      All parents must also complete the PLUS Application
      Process and a Federal Direct PLUS Loan Master
      Promissory Note at www.studentloans.gov

 2012–13 Federal Perkins Loan Documents

      To be filed by students who have been awarded a
      Federal Perkins Loan as part of their financial aid
      award.
