2012-13 Cal Grant GPA Verification Form

       For Undergraduate California Residents Only

2012-13 Dependent Verification Worksheet

       If you are a dependent student, and your federal
       Student Aid Report (SAR) indicates that you have    
       been selected for federal verification, please     
       complete this form and submit it to the M Center at
       Mills.

2012-13 Independent Verification Worksheet

      If you are an independent student, and your federal 
      Student Aid Report (SAR) indicates that you have
      been selected for federal verification, please
      complete this form and submit it to the M Center at
      Mills.

IRS Tax Return Transcript(s)

       For new and continuing students selected for federal
       or institutional verification after filing the FAFSA.

       Students selected for verification after filing the   
       FAFSA must submit to the M Center by May 15, 2012
       Federal Tax Return Transcripts if they choose not to
       use the IRS Data Retrieval Tool (DRT) or do not
       qualify to use the IRS Data Retrieval Tool (DRT) to 
       update their FAFSA with actual 2011 tax information
       after federal tax returns have been filed.  Copies 
       of federal tax returns do not meet this federal 
       government documentation requirement.

       Students selected for federal verification are notified
       on their Student Aid Report (SAR).  Students  
       selected for institutional verification will be notified 
       by the M Center.

       Dependent students selected for verification must
       submit IRS Tax Return Transcripts of 2011 tax year  
       information for the student and parent(s).

       Independent students selected for verification must
       submit IRS Tax Return Transcripts of 2011 tax year 
       information for the student and spouse, if
       applicable. 

       Note: Students who are not selected for federal or
       institutional verification are not required to submit
       copies of federal tax returns or IRS Tax Return
       Transcripts.

2011–12 Noncustodial Parent's Statement

       For New Undergraduates Only

       To be filed by the noncustodial parent in the case of
       a dependent student whose parents are divorced,
       separated, or were never married.  (The custodial
       parent should complete the FAFSA and Mills College
       Financial Aid Form.)  The Noncustodial Parent's
       Statement should be submitted to the M Center at
       Mills.

 2011–12 Certification of Non-filing form

       For Continuing Undergraduates Only

      To be filed by independent undergraduate students
      who are not required to file a 2010 federal tax   
      return and by the parent of a dependent     
      undergraduate student if the parent is not required   
      to file a 2010 federal tax return. This form should be
      submitted to the M Center at Mills.

 2011–12 Dependent Verification Worksheet

  2011–12 Independent Verification Worksheet

      If you are an independent student, and your federal 
      Student Aid Report (SAR) indicates that you have
      been selected for federal verification, please
      complete this form and submit it to the M Center at
      Mills.

  2011-12 Federal Direct PLUS Loan Request Statement  

      To be filed by parents applying for a Federal Direct
      PLUS Loan. This form should be submitted to the
      M Center.

      All parents must also complete the PLUS Application
      Process and a Federal Direct PLUS Loan Master
      Promissory Note at www.studentloans.gov.

  2011-12 Asset Verification Form

      Complete this form only upon request by the
      M Center. 

  2011–12 Federal Perkins Loan Documents

      To be filed by students who have been awarded a
      Federal Perkins Loan as part of their financial aid
      award.

  2011-12 Graduate Assistantship Contract

      Graduate students who have been awarded an 
      assistantship as part of their financial aid award
      should complete the first page of this document and
      return it to the M Center.
