Coordinating Care
Health Programs works closely with the University Health Services clinical and support staff to provide students with high quality, coordinated care. In addition to our efforts, students are encouraged to take an active role in their healthcare maintenance. Whether you are a SHIP member, or a registered Mills student receiving "Basic Medical Services", there are a number of steps you can take to ensure that you receive the best, most appropriate care.
Non-SHIP Members
- Know your medical insurance carrier and plan benefits
- Select a local primary care provider that accepts your insurance plan
- Keep a copy of your insurance card, particularly when traveling
- Know your medical history
- Provide a copy of your medical records to the UHS Medical Records department if you anticipate using the "Basic Medical Services" at the Tang Center
- Schedule routine medical and dental appointments
- Use the "Referral Worksheet" (pdf, 74k) to help you coordinate care
- Contact Health Programs with questions regarding care coordination
SHIP Members
- Know the primary SHIP benefits
- Select a primary care provider at Tang
- Select a primary dental provider through the MetLife provider network
- Know your medical history
- Provide the UHS Medical Records department with a copy of your medical records
- Schedule routine medical and dental appointments
